please note that Applying online will give you the option of having a provisional access card to a licensed dispensary. if you submit your application for mcpp before 6 pm cst, you can download the provisional card the following day after 8:30 am. this card is valid up to 90 days & only valid at your chosen dispensary location.
FILL OUT THE REQUIRED INFORMATION ON THE APPLICATION AND provide the following:
Submit a clear, color copy of an Illinois Driver’s License, Illinois State ID, or the photograph page of a US passport.
If your Driver’s License, Temporary Visitor Driver's License or State ID address matches your application submit one additional proof of residency. If you submit a US Passport as your proof of identity or your Driver’s License or State ID address does not match the address on your application, submit two PROOFS OF RESIDENCY SUCH AS PAY STUB, BANK STATEMENT, UTILITY BILL, ETC. fOR A COMPLETE LIST OF ACCEPTABLE DOCUMENTS, CLICK HERE.
All applicants, except for Veterans receiving care at a VA facility, must have their physician fill out a written certification for the use of medical cannabis.
You must scan the form and save it as a .pdf document to upload it with your on-line application or include the original document with your paper application. The certification must be received within 90 days of your application and must show an in-person visit date within the last 90 days.
tHE FOLLOWING PROCEDURES AND DOCUMENTATION MUST BE COMPLETED:
A Photograph MUST BE SUBMITTED THAT MEET THE FOLLOWING REQUIREMENTS (pASSPORT PHOTO VENDOR RECOMMENDED):
Taken in the last 30 days
Taken against a plain, white or off-white background or backdrop
In natural color (Do not use a filter)
Full-face view directly facing the camera with a neutral facial expression and both eyes open
At least 2 inches by 2 inches in size
a Check or Money Order MUST BE MADE to Illinois Department of Public Health. The reduced fee is for qualifying patients enrolled in the Federal Social Security Disability Income (SSDI), Supplemental Security Income (SSI) disability programs, or Veterans. Patients enrolled in SSDI or SSI – Submit a “Benefit Verification Letter” from the Social Security Administration that shows your name and address and the type of benefits that are received. This letter must be dated within the last year.
